| Short answer: Your modelines are wrong.

modelines are depreciated. More accurately, your refresh rate/resolution
combination is wrong. Example:

Section "Monitor"
~ Identifier "SAMSUNG"
~ HorizSync 30-70
~ VertRefresh 50-160
~ Option "dpms"
~ vendorname "Samsung"
~ modelname "SyncMaster 763mb"
~ DisplaySize 325 245
	Gamma 1.35 1.30 1.50 # created by KGamma
EndSection

RandR extentions to XFree 4.3 add the ability to change any of these on
the fly. XFree now also gets a great deal of information from the
monitor's DDC.
