Short answer: Your modelines are wrong.
You need to change the numbers on the modelines.
You are pushing the video beyond it's refresh
capability at the refresh rate and/or horizontal
and/or vertical frequencies.
Perhaps you don't have enough modelines to choose from.
I would run X -configure and have it regenerate
your XF86Config files. xvidtune can also help.
