NO NEED TO USE THE X-MONO SERVER.
Cards with the NCR77C22E are doing fine with the XF86_Color server
( I know because I'm posting this from a box with the a.m. card )

Either you are missing /dev/mouse or it is a soft link pointing to
nowhere.
If your mouse is on "COM1" make it /dev/ttyS0 in Xconfig.

I happens to me too. If I leave X the screen is scrambled.
