My mouse is a Genius GM-6 (3-button).
It has a Microsoft mode and a Mouse Systems mode.

I see the following different behaviors:

Test Mouse Mode Emulate3Button MB1 MB2 MB3
======= =============== =============== ======= ======= =======
1 Microsoft NO L NOTE 1 R
2 Microsoft YES L L+R R
3 MouseSystems NO L M R
4 MouseSystems YES ... NOTE 2 ...

NOTE:
        1. Can't get a MB2 from any combination of MBx.
        2. Never tried this (Test number 3 worked
           so their was no need to try anything else:-).

The moral of the story ... I've noticed that most people who are posting
about this 'no action until mouse moves' problem have:
        a) A three button mouse.
        b) They're running in Microsoft mode.
So, does anyone running a three button mouse in Microsoft mode get
Emulate3Button=OFF to work. If not, perhaps the XFree stuff just
doesn't believe that Microsoft mice can run anyway but 3 button
(after all, aren't all Microsoft mice 1 or 2 button?).
