Actually, it's Microsoft that doesn't support the middle button.
Microsoft doesn't make three button mice, and the 'Microsoft mouse
protocol' does not support it. Simply nothing happens when the middle
button is pressed on a three button 'Microsoft compatible' mouse, when
it is in Microsoft mode.
Returning to the original question: may I recommend the Logitech Pilot
serial mouse. I have one of those, and it uses an extended Microsoft
protocol, which adds support for the middle button. XFree86 knows of
this extension, so you can just put Microsoft "/dev/ttySX" in Xconfig
and off you go. No hassles with switching modes by "holding down the
middle button then-and-then" or whatever...
